PICU DRUG INFORMATION
Calcium
Electrolyte
Hypocalcemia IV: Calcium gluconate bolus: 50-100 mg of calcium gluconate/kg (usual maximum: 2-3 g/dose) Calcium gluconate infusion: 0.05-0.1 mmol/kg/h (usual maximum: 1 gram/h or 2.32 mmol/h to start) adjust rate Q4H Calcium chloride: 10-20 mg of calcium chloride/kg (usual maximum: 1 g/dose) Central line preferred.
PO: 50-150 mg/kg/day of elemental calcium divided QID
These doses are suggested starting doses, increase and repeat as required.

Calcium gluconate 10% = calcium gluconate 100 mg/mL = elemental calcium 9 mg/mL, 0.45 mEq/mL, 0.23 mmol/mL
Calcium chloride 10% = calcium chloride 100 mg/mL = elemental calcium 27 mg/mL, 1.4 mEq/mL, 0.7 mmol/mL
PO calcium carbonate:
Liquid as 80 mg elemental calcium/mL Chewable tabs (Tums) 500 mg (200 mg elemental calcium/tab
Oyster shell tabs 1 250 mg (containing 500 mg elemental calcium/tab)
PO calcium lactogluconate:
sugar-free liquid (20 mg elemental calcium/mL)
