PICU DRUG INFORMATION
Amiodarone
Class III Antiarrhythmic
Life threatening arrhythmias IV: 5 mg/kg (usual maximum: 300 mg/dose) , may be repeated for breakthrough arrhythmias. then: 5-15 microgram/kg/min
Administration: Give loading dose over 20-60 minutes for perfusing rhythms or rapid bolus for VF. Hypotension may respond to reducing the infusion rate. Central line required for concentrations >2 mg/mL.
Continuous infusion mixture: Central line:
<10 kg: 180 mg in 50 mL D5W (3.6 mg/mL)
10-39 kg: 900 mg in 250 mL D5W (3.6 mg/mL)
≥40 kg: 900 mg in 250 mL D5W (3.6 mg/mL)
Peripheral line:
<10 kg: 90 mg in 50 mL D5W (1.8 mg/mL)
10-39 kg: 450 mg in 250 mL D5W (1.8 mg/mL)
≥40 kg: 450 mg in 250 mL D5W (1.8 mg/mL)
Heart block requiring pacing has occurred. Do not use in cardiogenic shock, severe sinus node dysfunction, sinus bradycardia, 2nd and 3rd degree AV block.

Pharmacology: Class III antiarrhythmic but possesses properties of all 4 classes. (Class I - blocks sodium channels, Class II - antisympathetic activity, Class III - prolongation of cardiac action potential. Class IV - negative chronotropic effect on nodal tissues). Blocks potassium and calcium channels. Very large volume of distribution and long half-life.
